MCWA COMMUNITY OUTREACH RECOGNITION AWARDS Nomination Ballot - 2023

Dear MCWA Members,

This is the third year our organization will be recognizing the special contribution of two of our members who support our Community Outreach Program.

We need your input. Please complete this nomination form. You do NOT have to nominate someone for each category.

NOMINATION DEADLINE: JANUARY 25, 2023

Outreach Lifetime Giving Award Criteria: MCWA member who has “gone beyond” to help “GROW” our community outreach program for MORE THAN FIVE YEARS. Examples: creates vision, provides leadership, chairs fundraiser, super organizer, recruits volunteers and sponsors, etc.

MCWA Big Heart Award Criteria: MCWA member who has devoted hours of work and personal involvement in the past year or two with one or more of our charities.

If you prefer to fill out a paper ballot, you can find them in the Ladies Locker Room. You can snail mail them to Gale Mitchell in the self addressed envelope provided with the nomination form.

Anne Farnell, Mary Ann Meyer, Mimi Ianuzzi and Peggy Cutrino were recognized in past years and are not eligible this year.
